SummaryThe Central Texas Veterans Health Care System is seeking
a highly qualified Social Worker to serve as the Program
Coordinator, Dialysis and Organ Donation, The successful candidate
works at an advance practice level and is the point of contact for
inquiries regarding dialysis and organ donation services. This
position reflects the high degree of specialization, training,
experience, and expertise required to satisfactorily perform in

incumbent provides program coordination for the Dialysis Program.
Coordinators for the Dialysis Program ensure that the specialized
needs of the Veteran at or near End Stage Renal Disease (ESRD) are
thoroughly assessed and effective treatment plans are implemented
that fit the physical, emotional, and psychological needs of the
Veteran. Duties and responsibilities of the incumbent for the
dialysis program include but is not limited to processing
authorizations for Veterans newly referred for outpatient dialysis;
establishing dialysis in the community within 20 miles of the
Veteran's home; processing requests for transient dialysis for
Veterans who are traveling; providing education to staff and
Veterans on ESRD to include psychosocial issues such as location of
dialysis centers, scheduling, payment of outpatient dialysis, and
the VA Transplant Program; maintaining short-term case management
to ensure the Veterans' needs are being met while on dialysis;
maintaining contact with the dialysis centers in the catchment area
for CTVHCS which may involve periodic travel to the dialysis
centers; maintaining a detailed registry listing all Veterans with
ESRD who are followed by nephrology and who receive outpatient
dialysis; meeting and communicating as a vital team member of the
nephrology-dialysis team to explore needs of Veterans on dialysis
and needed changes to improve services within the Dialysis Program;
coordinating yearly renewals for outpatient dialysis with
nephrology; and monitoring trends to analyze needed changes within
the Dialysis Program.The incumbent provides program coordination
for the Organ Donation Program. Coordinators for the Organ Donation
Program ensure that the medical center's organ donation program
runs efficiently. Duties and responsibilities of the incumbent for
the Organ Donation program include but is not limited to providing
education as point of contact to Veterans and medical center staff
regarding organ donation including written materials and brochures;
providing education on organ donation to new nursing staff;
collaborating with organ procurement agencies and medical center
staff regarding organ donation and procurement; maintaining
availability to assist with issues that may arise during the
notification to procurement agencies, interaction with family
members, and procurement; meeting with Veterans referred for
potential transplants and completing the VA Transplant Psychosocial
Assessment to be included in the transplant referral packet;
collaborating with the Transplant Coordinator regarding detailed
listing of Veterans on the transplant waiting list; completing the
monthly death report from the procurement agencies; providing
quarterly reporting on organ, tissue, and eye donation data to the
Clinical Executive Council (CEC); maintaining the Memorandums of
Agreement (MOAs) with procurement agencies and ensuring the
procurement agencies follow the guidelines as mandated by the MOAs
and federal government; and monitoring trends to analyze needed
changes within the Organ Donation Program.NOTE: Incumbent must have
a current, valid, unrestricted driver's license and the ability to
operate a GSA vehicle for visits to community dialysis centers and
for on-site educational activities for the Dialysis and Organ
Donation Program at all CTVHCS sitesWork Schedule: Monday-Friday

school of social work fully accredited by the Council on Social
Work Education (CSWE). Graduates of schools of social work that are
in candidacy status do not meet this requirement until the School
of Social Work is fully accredited. A doctoral degree in social
work may not be substituted for the master's degree in social work.
Verification of the degree can be made by going to to verify that
the social work degree meets the accreditation standards for a
masters of social work.Licensure. Persons hired or reassigned to
social worker positions in the GS-0185 series in VHA must be
licensed or certified by a state to independently practice social
work at the master's degree level. Current state requirements may
be found by going to Provision. May qualify based on being covered
by the Grandfathering Provision as described in the VA
Qualification Standard for this occupation (only applicable to
current VHA employees who are in this occupationPhysical

Occupational Health Services.English Language Proficiency.
Candidates must be proficient in spoken and written English to be
appointed as authorized by 38 U.S.C. -- 7403(f).Grade
Determination: Senior Social Worker GS-12Experience/Education. The
candidate must have at least two years of experience post advanced
practice clinical licensure and should be in a specialized area of
social work practice of which, one year must be equivalent to the
GS-11 grade level. Senior social workers have experience that
demonstrates possession of advanced practice skills and judgment.
Senior social workers are experts in their specialized area of
practice. Senior social workers may have certification or other
post-masters training from a nationally recognized professional
organization or university that includes a defined
curriculum/course of study and internship or equivalent supervised
professional experience in a specialty.Licensure/Certification.
Senior social workers must be licensed or certified by a state at
the advanced practice level which included an advanced generalist
or clinical examination, unless they are grandfathered by the state
in which they are licensed to practice at the advanced practice
level (except for licenses issued in CA, which administers its own
clinical examination for advanced practice) and they must be able
to provide supervision for licensure.Demonstrated Knowledge,
Skills, and Abilities. Candidates must demonstrate all of the
following KSAs: (a) Knowledge of program coordination and
administration which includes consultation, negotiation, and
monitoring. (b) Knowledge and ability to write policies,
procedures, and/or practice guidelines for the program. (c) Ability
to supervise multidisciplinary staff assigned to the program. (d)
Skill in organizing work, setting priorities, meeting multiple
deadlines, and evaluating assigned program area(s). (e) Ability to
provide training, orientation, and guidance within clinical
practice.Assignments. For all assignments above the full
performance level, the higher level duties must consist of
significant scope, complexity (difficulty), and variety and be
performed by the incumbent at least 25% of the time. Program
coordinators at the GS-12 grade level are administratively
responsible for a clinical program providing treatment to Veterans
in a major specialty area, such as but not limited to: Spinal Cord
Injury, Homeless Continuum Veteran Program, Hospice and Palliative
Care Program, Suicide Prevention Program, Veterans Justice
Outreach, Caregiver Support Program and Community Nursing Home
Program. The program coordinator may be the sole practitioner in
this specialty at the facility and typically provide direct patient
care services in the program area. The program coordinator oversees
the daily operation of the program, develop policies and procedures
for program operation, and prepare reports and statistics for
facility, VISN, and national use. They may be responsible for the
program's budget, developing and monitoring staff compliance with
practice, standards and guidelines on documentation, workload, data
entry, ethical practice and service delivery. The program
coordinator provides analysis and evaluation of clinical program
data and computerized programs to identify system-wide trends and
needs to enhance the quality of service. They may be responsible
for, or contribute to, the program's resource and fiscal
management, monitoring control points developing the annual budget,
