Job Description:
RN Hospital at HomeOverview The ACRN is responsible for starting
the patient's care which includes nursing assessment, medication
reconciliation, safety assessments, and implementing standardized
protocols. Responsibilities Participates in virtual admission
huddles and performs timely start of care visits for new admissions
to the program. Performs comprehensive patient assessments and
provides patient care. Performs medication reconciliation in the
home at admission and discharge. Participates in virtual rounding
with treating physicians and recovery care coordinators.
Administers prescribed medications (typically intravenously),
obtains blood samples, and provides therapeutic interventions.
Educates patient on how to manage their illness, prescribed
medications, nursing interventions, and care plan. Participates in
the coordination of services with Clinical Manager, Recovery Care
Coordinator, other disciplines, and community resources as needed.
Identifies and acts on changes in patient's condition. Evaluates
and revises care plans/assessments based on changes in patients
and/or their environments. Documents all assessments,
interventions, and care coordination details. Administers
medications including IV medications as ordered. Inserts foley
catheters as ordered. Utilizes and instructs patient/family on the
use of patient monitoring tools used for biometric data capture,
patient symptom reporting, and virtual visits. Participates in
regular, in-service, or special case orientations as needed.
Participates in on-call rotation. Complies with policies,
procedures and regulatory mandates including but not limited to
abiding to the terms of the Amedisys Compliance Program. Performs
other duties as assigned. Qualifications Required- Associate of
Science in Nursing. Current, unencumbered state license to practice
as a Registered Nurse. Three (3+) years of adult acute inpatient
medical, or medical/surgical care experience within the last five
years. Current CPR certification. Valid Driver's License. Reliable
transportation and company required liability insurance.
Proficiency with general use and functions of the computer, such
as, usernames, password concepts, internet, email, navigation of
computer desktop, including starting programs, using files, and
windows, effective use of navigation buttons and tool bars, ability
to self-manage online HR services and online training programs.
Preferred- Bachelor of Science in Nursing. Experience providing RN
services in the home, community, or clinic. Adult medical critical
