Job Description:
Job Description Baylor Scott & White Hospice is a collaborative,
interdisciplinary team of professionals providing end of life care
for patients and support for families in their homes. Nurses, Home
Health Aides, Case workers, Chaplains and Volunteers all work
together to provide professional compassionate care. Nurses have
autonomy, no office- start and end from home, limited call, no
weekends (except weekend positions) 12-16 patients per RN with

Summary The Registered Nurse on Unit is a key player in patient
care management, following our nursing practice model. They work
with other health care staff to deliver patient care effectively.
Our Registered Nurses use the nursing process efficiently. They
diagnose patient responses to health issues and create care plans
accordingly. It includes evaluating healthcare needs and responses.
Essential Functions of the Role - Prioritize needs. Create,
implement, and assess care plan to achieve outcomes. Adapt plan as
necessary. - Observe indicators closely, support evidence-based
changes with research and hands-on learning. - Create a caring,
safe environment for patients, families, and staff. - Build
relationships with boundaries in mind. Focus on managing emotional
and physical pain to promote healing and reduce suffering. -
Facilitate learning for all members of the healthcare team,
including patients, families, and nursing staff, ensuring everyone
is well-informed. - Collaborate effectively with colleagues and the
community to develop and execute an integrated plan of care. - Use
problem-solving strategies and utilize available resources to
support patients, family and staff as part of a systems thinking
approach. Key Success Factors - Familiarity with nursing and
patient care standards and procedures. - Awareness of legal
requirements and standards of certification and accreditation
bodies, including the specifics of the hospital and respective
department/unit. - Good command of medical terminology, with a
focus on healthcare promotion, risk reduction, and disease
prevention. - Effective communication skills, along with social
aptitude for interacting with diverse stakeholders. - Possessing
critical thinking and problem-solving skills is crucial. -
Capability to identify and report changes in patient conditions,
develop, and modify patient care plans as required. - Basic
computer skills necessary for day-to-day assignments. Belonging

RN program. - Field of Study: The applicable major should be
nursing. - A minimum of 2 years in a relevant role is expected. -
Certification: Hold a BLS certificate now or obtain within 30 days
of starting in this role. - Registered Nurse License - Highly
prefer Hospice or Home Health experience
