specializing in total joint replacement (TJR) and sports medicine.
Are you ready to merge hands-on clinical excellence with strategic
patient pathway coordination? BONE DRs Orthopedic Care is seeking a
highly motivated and exceptionally organized professional to fill a
unique dual role: Physical Therapist (PT) and Joint Replacement
Coordinator. This is not a standard PT position. You will serve as
the crucial link between our orthopedic surgeons, patients, and
ancillary care providers, ensuring seamless, high-quality care
delivery from pre-op education through post-op recovery, while
maintaining an active clinical physical therapy caseload. I. The
Role: Clinical and Coordinative Excellence Physical Therapist
Responsibilities ? Provide advanced, evidence-based orthopedic
physical therapy treatment for Operative and Non-Operative Patients
? Conduct comprehensive evaluations, develop individualized
treatment plans, and manage patient progression. ? Collaborate
directly with the orthopedic surgeons to optimize surgical outcomes
through specific, tailored rehab protocols. ? Utilize in person and
remote monitoring systems to optimize patient compliance and
promote increasing positive outcomes Joint Replacement Coordinator
Responsibilities ? Patient Education & Navigation: Serve as the
primary point of contact for all TJR patients, conducting
pre-operative education classes/sessions, setting expectations, and
answering non-clinical questions related to the surgical journey. ?
Pathway Management: Coordinate the entire episode of care,
including pre-admission testing, scheduling, equipment needs (DME),
and discharge planning (home health, skilled nursing, etc.). ?
Quality & Metrics: Track key outcome metrics (e.g., length of stay,
readmission rates, patient-reported outcomes) and report
performance data to the physicians and management for continuous
quality improvement. ? Resource Development: Develop, update, and
disseminate standardized patient education materials and surgical
pathway protocols. II. Qualifications & Credentials Required: ?
Doctor of Physical Therapy (DPT) or Master's Degree in Physical
Therapy from an accredited program. ? Current, unrestricted
Physical Therapist license in the State of Texas. ? Minimum of
three (3) years of clinical experience in an outpatient orthopedic
setting, with significant experience managing total joint
replacement patients. ? Exceptional organizational skills and
demonstrated experience in project management or care coordination.
? Proficiency with Electronic Health Records (EHR) and Microsoft
Office Suite. Preferred: ? Board Certification in Orthopedic
Physical Therapy (OCS) or equivalent specialization. ? Experience
in developing or teaching patient education curricula. ? Bilingual
proficiency (English/Spanish) is a plus. III. Why Join Our
