Summary: The Care Coordinator holds a vital position at The Annex,
serving as the primary liaison between clients, their families, and
external professionals involved in their comprehensive care
coordination. This role is responsible for guiding clients through
their entire treatment journey, providing consistent support,
coordinating care initiatives, and advocating for client needs as
appropriate. The Care Coordinator ensures continuity of care by
diligently managing discharge planning and connecting clients with
essential resources upon program completion. This position is
instrumental in facilitating clients long-term recovery by
establishing linkages with outpatient providers, supportive
services, and community resources tailored to individual
requirements. The Care Coordinator must possess the necessary
experience, education, and capability to deliver clinical
interventions and de-escalation strategies when required. This role
reports directly to the Operations Director and indirectly to the
Clinical Director. Supervisory Responsibilities: This position has
no direct supervisory responsibilities but collaborates with
clinical teams and external service providers to support client
needs. Detailed Responsibilities: Complete initial case management
assessment for all clients within one week of admission. Create
individualized discharge/aftercare plans. Facilitate referrals and
linkages to community services. Maintain accurate documentation of
case management activities including weekly case management
sessions. Maintain timely, professional, and confidential
communication with referents, outside treatment providers, and
other involved professionals to support continuity of care and
collaborative treatment planning. Engage loved ones and family
members in accordance with The Annexs communication protocols to
promote support, transparency, and clinical alignment, when
appropriate and with proper consent. Advocate for client access to
services. Collaborate with multidisciplinary teams. Provide
clinical interventions if/when necessary. Monitor client progress
post-discharge. Required Knowledge, Skills, and Abilities:
Knowledge of community resources and aftercare planning. Strong
advocacy and client engagement skills. Understanding of mental
health and substance use disorders. Crisis intervention and
de-escalation techniques. Documentation accuracy and EMR
proficiency. Problem-solving and crisis management skills.
Organizational and time-management proficiency.
Education/Experience: Bachelors degree preferred. 2 years of case
management experience preferred. Valid Texas Drivers License
required. Specific certifications related to mental health or
substance use counseling (e.g., LCDC in Texas) are a plus.
Experience working in a residential treatment center, behavioral
health facility, or similar setting preferred. Familiarity with
local community resources in the Austin area is beneficial.
Proficiency in relevant software and EMR systems (specify systems
if applicable).
